Userform with Login and Password (with limited number of entry for password)

Hi Fellas,

Actually, I was wondering if this is possible.
I have created a userform for two textboxes (one for username and one for password). I want to give access to wrong password or username for only three times.
And if someone is entering wrong credential for more than three times (obviously 4th time), the userform should be unloaded.

I expect, some VBA masters can help in this..
