Good tutorial for Userforms?

I'd like to create a userform with three radio buttons and one text box.

The buttons would pick one choice based on previously set variables (displaying next to the related button).

If none of those choices is what I want, I could fill in a textbox and use that.

What is the best tutorial for user forms?

(I will post what I come up with when I figure out how to ask the right question, then build the code/form.)
