Auto Report

Hello there,

I am working on a spreadsheet that can automatically create a report and was looking for some input.
The report will take certain input data in sheet1 and compile it in a table on sheet2 . My data is currently in a large excel spreadsheet. I have attached a snippet of that data along with an example and description of the report.
