match up names to extract num and type from another sheet

Dear all,

I am writing to seek help, in creating a automated function/macro to extract each name's num, type, id and date from one sheet to another.

Sheet1 and Sheet2, has input data (actual data is 10,000 lines). I would like to be able to extract each name's num by matching up the names from sheet1 and sheet2 and extracting each name's all num as shown in sheet3.
