Radio Buttons/Check Boxes and Excel

It is a great solution, but I am unsure how to add checkboxes and radio buttons. If anyone can guide me to a tutorial or an example I would be happy to learn!

Seems that I am getting a value of N/A.

Also I noticed the form values should be numbered instead of all "2's" like this:

What's in the worksheet? The code you posted assumes that there will be a column header for each field in sheet1 in cell a1

I couldn't even get vbscript running in IE11 - though it's still supported apparently, so here it is with JavaScript (tested and working)

It's posting all of the data as needed, but when adding a new entry it overwrites previous entries as well as repeats the last question.
I appreciate all of the help so far.

As a side note I am saving the html code as a .hta file. It shouldn't cause any problems though.
