    Use a form to Popualte a Data Base

    I have a Request form with both check boxes and text that will be completed by a number of people.
    I would like to take the information from the Request form and place it into a data base; into rows where I can see a running list of the request information.
    Does anyone know of a example of something like this?
    Can this be done thru Macros or will it need to be done thru VBA?

    It would really need to be done in Access

    I don't have Access so I'm pretty much stuck.
