Unscrambling a data base help needed

So I am trying to unscrew a cvs file that come out of an ACT program written to manage subscriptions. The Act file had the ability for a customer to have up to three subscription in his "record" so basic fiields had a Start date 1 start date 2 start date 3, end date 1, end date 2 etc etc.

So the attached mockup sample shows the the output to a cvs file.

the attachment also shows what I am trying to do. Basicly get it translated into a simpler start, end, cost, code data base.

I don't even know where to start ? Macros, VB code, etc.

Help appreciated.
