Sum one column meeting criteria from two other columns

Am trying to add cells in one column if the criteria matches info in two other columns. I attached my file [excell 2003] I had to shorten the list to fit the requirements but I can change that in a formula. I am in over my head but you can see what I tried, with someone's help yesterday, but maybe we can use the data in the summary page info. I don't know, please help
