extract data based on four criteria in excel sheet or VBA code to extract

Please Help me ,I need to extract data based on input in two cell and extract two data with four criteria please loo at the example,help me!!!

Need to retrieve data column A and column C data based on
1)week no
2)date in that particular week
3)column C data to be selected based on green indiaction


for example:

WEEK NO IF IT IS WEEK 14
DATE IF IT IS DATE 2

RETIEVED DATA SHOULD BE AS FOLLOWS
PUMP TAGS A OR B
P-001 A ( SINCE Cell 2R indicates Green color)
