    You can set this up manually for one row, and then copy that row down to the bottom of the first printed page, then you can copy that form down the sheet as many times as you like. You will need to manually merge the requisite columns on the first blank row of your form (it's difficult to say what row that is from your picture). It looks like cells B to G need to be merged, then H to M, then N to Q, and so on across the row. You might need to adjust the row height to allow for handwriting being larger than a printed line. Use Format cells to put borders around each cell in that row, then you can just copy that one row down as far as you need to.
