Combine two tables to one

We are still struggling and would really appreciate any help. We haven't been successful applying code to solve this ....

We have two tables that we want to combine to form one table. The format is identical for all tables (see attached file).

Maybe someone has a clever solution??! Thank you in advance for any help!
