    Conditional Drop Down Menu

Merry Christmas everyone. I'm hoping Santa can help here. what I want seems simple but I keep tanking it. I want a drop down menu with selections A, B & C. Then, when one of those is selected you get the options relative to each. So A would bring up 1, 2 , 3 & 4. B would return 5, 6, 7 & 8 and so on. The fourth option has to be a blank in case the user wants to enter their own information. Any ideas?
