Undoing pivot tables

Hi,

I'm an amazon seller. Amazon has a tab delimited file that we can download in order to view all the different fees that amazon charges for each sale. The problem is that it comes in pivot table and I want to put the information in columns. Having everything in columns should help me in uploading the information to accounting software such as quickbooks. I uploaded a sample to better explain what I need to do. Any and all help would be greatly appreciated.
