add new page in the same sheet with a same format as the 1st page and so on

Hi all,
So here what I want to do, I have one sheet with the table so someone can enter the data in the colum provided refer to the example file. When someone enter the data at the end of the 1st page in the same sheet by default it will add new page. But I want the 2nd page and so on is the same page with the 1st page Name and class should be there also, in other word I can manually copy paste the 1st page into the 2nd page and so on but it will burn me out because more sheet like these I have to create. Is there a way or vba code so the new page will add automatically just like I mention before. The example file available in the attachment
