There is no reason you can't build your own template since Excel is EXTREMELY user friendly for this sort of thing. Furthermore, I would not be worried about building something this simple thinking that you might get pigeon holed as your needs change for the template, because it is also very easy to modify an existing template.



My advice: start by deciding what you want in the template. Mock something up and build it one step at a time. If you hit roadblocks, ask for some help. If you prefer, you can grab something someone else has already started then modify to your needs.
