date referenced data

I have a student testing data report that has multi information relating to various tests and dates of tests. I would like to collate these into one page for each year. How can I put a formula in where it will vlookup an id number then says if date is between 1/1/2012 and 1/6/2012 and test symbol (RR) matches cell (b3) then put column 6, all in one formula.
I have attached a trial of the testing spreadsheet.
The format of the data page I cannot change.
