I really need help, I have been trying to figure this out for week's and have gone over this forum to find a solution to no avail. If you can help please provide step by step instruction...I would be eternally grateful. Please see attached excel sheet that breaks down what I'm looking for. Thank you in advance!

What kind of MAILMERGE are you trying to do? Your data as it stands is the standard format for doing a MAILMERGE.
