Information Extract

Hi

I want to extract data from one sheet by searching by Department and copy onto sheet 2 as indicated under the field names. I have tried using Pivot tables and Vlookup functions but to avail.

I have attached a sample of the spreadsheet with data in it and on sheet2 the information I would like to see.

I appreciate all the assistance I can get in this regard.
