Trying to gets displaying as dates when returned by formula (not displaying as 41000, etc)

Hi Forum,

I am trying to provide a title for a chart that is generated by a formula in a cell. I want the title to look like:

Scores by Month - Apr-12 to Aug-12

The cell contains the formula:

="Scores by Month -"&" "&O11&" "&"to"&" "&O12 and refers to two cells (O11 and O12) that contain dates.

The problem is that it appears on the chart as:

Scores by Month - 41000 to 41122

How can I get the 41000 and 41122 to display as Apr-12 and Aug-12 (or whatever dates appear in the cells)?

I have played about with Format Cells but had no luck thus far.
