Show 'some' results from large set of data from corresponding points

Hi there,

I have a very large set of data to deal with (attached file is a basic concept of the contents). My data comes in sets of 4 columns, and the important points from each set is 4th column. What I am looking to do (and can't!) is the following:

In a new column (or more if required) have a result that gives the column Heading if column 4 has the entry 'Off'. That is if column headed P4 has 'off' in it in a particular row, then the result cell will simply says 'P3', the next column would give the result relating to the off but from column 'S3', and a 3rd column with the result again relating to the result 'off' but from column R3. This may occur up to 5 times per row (potentially creating a new 15 columns).

I will have around 30 of these 4-column blocks but a maximum of 5 blocks per row will have entries; the spreadsheet I'm using will have around 1000 rows. I have attached a sample sheet - with the result I'm looking for at the end of rows 9 and 11.
