Without breaking any rules I am allowed to suggest a new title. Try "Lookup problem".

From a security point of view, (again I am not breaching any rules) I think the first thing you should do is amend line 1 of the s/s to something fictional as you do give a lot of personal information out which the people might not like publishing. Ideally, the names should be fictional but I can see it would be difficult.
