    multi-criteria search lookup fail

    I thought myself pretty capable with Excel but this is the first time I have ever posted a question that couldn't be answered by Google. I'm thoroughly stumped.

    I am basically trying to do a two criteria lookup from one tab to another. I have used both an AVERAGEIFS (data doesn't need to be averaged but it's a quick and dirty way to do a multi-dimensional lookup) approach as well as your standard MATCH/INDEX strategy.

    Both approaches work--until I added a new row of data to the "to be looked up from" tab. The old data is still able to be looked up. But no matter what I do or how I've tried, I cannot get the new row of data to show up properly.

    I've tried changing data formats, looking for apostrophes, spaces, anything that might mess up the lookup...but no, nothing is different from the old entered data to the new entry to be looked up.

    I have attached the file in question. Help!
