formula to choose top 5

Hi!
I'm trying to find a formula that will choose top five values for each period, and provide me with assigned information.

Please find an example attached.
The values are presented in columns I-K.
The fields to complete are marked in yellow.

Any ideas?


P.s. Once it is done, then there is other part of the question, which may be more challenging.
It may be the case, that some of top 5 will have the same values, in which case I would like an alphabetical order based on the name.
