Extract Data Based on Multiple Criteria

I am trying to extract data from a spreadsheet on my company intranet based on 2 criteria and display it in my own spreadsheet.

For example in the attached workbook if I wanted to lookup Ian Barry and only display his data.

The company spreadsheet has quite a bit more data in it.

The source spreadsheet is a bit long winded to access on company's intranet.

Was trying to find a clever way to automate the process.
