    Alteration to VBA Code

Can anyone help at all, I have the below codes (Write data to Worksheet & Load Data) which works fine, but I have added Text Boxes and taken away check boxes from my user form, but of course now the code needs alterations and I can not work out what needs to be changed, below are what each of my columns are in the excel spreadsheet that the code needs to write too. I have taken the two check boxes off the form and when i deleted that part on the code it did not work, I am a newby at all this so anyones help is greatly apprecaited.

Column 1 To 12 Text box
    Column 13 & 14 ComboBox
    Column 15 Text Box
    Column 16 & 17 Text Box
    Column 18 ComboBox

Could do, but I would put it in its own procedure and call that from both places.
