Index, Match, Left & or Replace needs refining

I have spent ages trying to solve this for myself but I am back seeking help from the experts.

Problem area is in R42:S51 of attached

Unfortunately I had been too hasty in thinking this was sorted out. The Left & Replace options I was given by TMS & Haseed A on here did exactly as requested.

Problem is with my initial part of the Index Match, I only tested it over a few cells and it seemed to be ok. However when I applied it over more cells I realised there was a flaw. I can see my mistake & the need to add in Small Row & Rows part to it but I am struggling to get it to work.

It was suggested a Pivot Table would be better suited to what I am doing (you guys should know) but as I have got this far and it is the final piece of the puzzle I want to persevere.
