Populating data from one sheet to another

Hey guys,
So I want to populate some cells to have the data from one sheet to the next based on if it has a "Y" then it populate. You will probably understand more when you look at the example.

So I need a formula that will look up Y and take the necessary values and place them in the reports sheet.

I attached an example.

Hello WGN,

Try this ARRAY FORMULA (must hit CTRL+SHIFT+ENTER, rather than just ENTER) in reports B4, copy down to each section.

Hey I tried the formula it worked fine but there is just one thing its giving me duplicate values....I attached the workbook with what I did...Please help...Thanks in advance.Book1.xls
