Matching data form multiple columns

I want to match data from three columns and display the corresponding value.I have attached a sample sheet.

Cells A1:D10 is my base data and when i enter data in A15,B15 & C15 .D15 should be filled in automatically by matching the entered data with the base data.

I used index and match formulas for matching two parameters but with three of them ,i am stuck.
