Index Match Problems

My dilema is pretty hard to explain via post, but in short I am trying to figure out how to use a row reference nested in an index(match) formula or a vlookup formula. I have provided an indepth explanation in the attached sheet. I have been trying to figure this one problem out off and on for the past couple days so if anyone could help me find the solution I would really appreciate it.
