Pivot tables

Hi All,
I am not too familar with the pivot tables. I am trying to bring a Initial Cost and Additional cost as well as the category, so all the data correspnds to appropriate fields but it doesn't seem to work. I attahed the file for review. Also, if it works, please provide the solution not just an answer. Thanks.
