Hey guys,

I am looking to create a drop-down box, which will have 6 different options:
Medical, Student, Job Loss (when either of these three are selected another box should show free.)
or
Travel, Winter, Misc (when either of these three are selected the box should show $10.00)


What is my best way going about this?
