Mail merge from duplicate data

I have a worksheet of many different addresses, some are duplicates others are unique. What I want to do is create a mail merge from the addresses that appear at least 5 times or more without mailing an address 5 times.

I have been using a pivot table to visualize the duplicates in a unique table only to help condense the data, however, I am unable to get cells in the pivot table to pull row data from a different worksheet in order to mail merge.
