excel table into a clean form

I have a pricing book for our company (part is attached) it has car bumpers organized by make, model, year, part, and price. I'm trying to build a form of some kind to take this data and put it into a clean readable way to print it out. I would also like this that i can use an excel spreadsheet to enter the data if something changes. That way when a price changes I can just update it and not have to retype it out into pages. The way I need it to work I just do not know where to start. For the form I would like it to group things IE place the Make of the car centered over the data one time instead of on each line. Any help would be great!
