I'm afraid I don't understand your data sample at all. Why is D6 not red? January 2011 is in the past, isn't it?
Why is D8 yellow? That date is over a year in the past, not within 30 days of today, and later than the day/month in C8.
How come that the dates in column D are different days than the dates in column C?
What is HIPAA Birth Month?
Are the dates in column C the people's real birth dates?
Do you want to compare the month of column C against the month in column D? Or against the current month?
I'm sure your question can be solved with conditional formatting and three simple rules, but I'm confused as to what these rules are, because your data is not consistent and the highlight colors don't match the description.
