    help w/formulas (index, counta, and choose)

    Hello all -

    The attached spreadsheet is a sample of my dataset. I received help from this forum in putting the formula together to calculate the values for the format I need for column G. What I can't figure out is why I'm getting an error in cell G2. I'm sure its very simple for the group here ... can someone please take a look at this?
