    Index, Match, Vlookup, or Offset?

Can anyone suggest a formula that might work for my spreadsheet?
I have a database tab that has rows listing routes and columns that list the speeds or types of routes.
On the circuit activity sheet, I want to be able to choose the type of circuit, such as a DS1, and have it fill in the cost, based on the locality of the route.

I am able to get all the cells to populate except one. The attached spreadsheet will hopefully show more of what I mean as it is difficult to describe exactly what my issue is.
The database sheet does all the calculation to provide all the rates based on miles, and percentages of ownership on the routes. So I basically just want to have the cells indicated, return the results from the database page.

I have index and match used in one cell, which works perfectly. I have vlookup in another that also works perfectly. But I can’t seem to figure out what will work in the empty cell shown in the spreadsheet. Sorry to sound so vague, but it is hard to explain.

wont just changing the index range work?
    =IF(B2="","",INDEX(Database!C$2:$N$39,MATCH($B2,Database!$A$2:$A$39,0),MATCH($A2,Database!$B$1:$N$1,0)))
