cell dependant on other cells

hi guys

i have an issue that i cannot get my head round

in cell H15 i have this

but what i need to do is i cell H5 has the phrase "with survey" in it i want it to read different

for example

D26 = sfk69 (4)(5) sfk 70 (4)
H4 = 5-7 weeks
H14 is dependant on whats in m14 but will read something like "( 03/12 - 17/12 )

so if cell H5 has supply only in it i would like it to read
sfk69 (4)(5) sfk 70 (4) 5-7 weeks ( 03/12 - 17/12 )

however if cell H5 has the phrase "with survey" in it i would like it to read
sfk69 (4)(5) sfk 70 (4) 5-7 weeks AFTER SURVEY

is this possible
