macro - userform issue

hello

managed to write a macro, but having some difficulty executing it!

i can try to explain it, but it wont help unless you see a file, so i have attached it.

basically, im wrote the macro to create a userform box so when users open this workbook, they can "sign-in" with their username, initials and date. this way i dont have to chase them to confirm the file they have sent......if someone can pls help me, its rather urgent. thx you.

- i keep getting a "object required" error in vbn and im not sure what im doing wrong, thxs

is there a reason why the popup box or userform box is not appearing?!! pls help txs
