User Data Form

Hi I am trying to create 2 userforms to enter data into my stock control sheet, i need the 1st form to have entry fields for -

REQUISITION NO.
NAME
DATE

I also need the form to enable me to enter upto 10 products and their quantites with a submit entry button, a clear form button and a close form button.

I need the second form for deliveries and returned stock, fields as follows -

ORDER / RETURN No.
NAME
DATE

the sheet that needs to be populated is attached, the fields activate lookup data information - the white sections are the lookup autofill parts, the blue, red and green parts are user entry.
