How to v lookup & use if statement?

Ok this is the problem I have.

I have posted a sample.

I need to fill in the data for week 8 and nine and the data to be included is from col N-Qso basically the unique identifier is the shop no, col A with raw data col N&P.

In Col O&Q is the raw data which gives a score, this then needs to go in col E-K(what will be L&M for wk 8 and 9) but using the abbreviations A=avberage,G=Good,NR= No report, P=Poor, Blanks I have been told =NR also but can be left blank.

Any help would be great as i'm really stuck
