Displaying cells greater than a number

I have a subtotaled sheet that I would like to lookup the cells whose sums are, for example, greather than $5,000 and have them all displayed in my budget sheet. Could someone help?

If possible maybe also display the subtotal name along with the amount.

I would like to extract the data from column d on a new sheet showing only the cells greater than 4000.
Possibly be able to vlookup to show the first and last name instead of a total at the end?
