Excel 2007 : School Survey Problem

HELP!!!
I Do not use Windows that much and I need to use Excel I have put my Data in, and I need to make a table,
I'm a bit Lost I have tried to make a table and when I do only one thing shows up from the question and one from the answer of the person,

It will bundle up all of everything,

Here is what it did, ( I attached it to the Post)

I do not have everyone, in yet, I have about another 150 to go,

I just would like to know how to map the "***" the Grade and each question together,
This is making me go crazy at the moment

Also I Do know That there are some light changes with words like none, or nothing, I am very tired and going to change those in the morning, thks
