Custom toolbars

This is probably really simple, but seems to be giving me some problems.

I wish to create a custom tool bar with my own buttons assigned to macros. I can do this. But how do i ensure that this custom toolbar only opens with that workbook, at the moment it opens in every excel workbook i have
