Button programming

Need some assistance in coding a button:

Click Open Chemical
Asks which chemical (Validates with a list if present) - if not says invalid
Ask mfg expiry date (Need data entry for 4 digit (display format mm/yy)
Asks for a bench/location (Dropdown list)

When everything is entered
Inputs current time as open time
Retrieves info from database @ shelf life
Starts clock on remaining time
calculates use before time
Prints out a label with open & expiring time
Stores data in the form of a table as attached.
Changes colour for time remaining (as time closes on expiry)

Would I need to create a form?
