IF AND Formula (possibly)

Hi,
I'd really appreciate some help with some formulas for populating three columns of data. I tried using the IF AND formula but I can't seem to get it workin, possibly because it may not be the correct option to use. In the attached excel file I have a sample and description of what I need to do but I have reached the limit of my excel formula abilities and would be grateful for any insight.
