using userform to create a new worksheet

can anyone point me in the direction of help in creating a userform to create a new worksheet

the userform needs to be used to name the sheet

and the userform will have a variable (number of policies, will be between 1-9)

this userform needs to be paired with a template sheet to create a new worksheet.
