Code needed to replace formulas and conditional formatting

Hi, I am still working on this sheet (attached) which details dates when a certain form has been filled in for a client.
I've attached the file and as you can see I've managed to put in formulas to figure out which 'time window' the date falls into and conditional formatting to highlight the ones which don't fall into the right windows.
However, this isn't enough - I need to know for each client which time windows they have 'hit' and which (more importantly) they have 'missed'.
Using the RAW sheet I have been trying to figure out some code, maybe using 'for each' or 'do while' but I'm getting nowhere fast.
Basically after the TOP Anchor Point, there needs to be a form filled in initally wihtin 5 weeks, then between 5 and 14 weeks, then within every 12 weeks after that. The lookup sheet may make it easier.
Any help would be very gratefully received, I have only just got back into writing/recording! code and need an expert!
    Thanks so much
