how can this be done?

i wana set up a questionaire with some checkboxes and text fields
and wen you hit the sumbit button.. it wud get added into another sheet as a record in one row and it would jus keep recording down the rows... any ideas to go about this ?? your help is greatly appreciated.. thanks
