retrieve filled cells and their date

I have attached a spreadsheet that needs a formula.

I need the formula in the summary page that will go to the scores page and look at the grid next to agent 1(C2:E32), find the cells that have a score in them and return that score along with the date of the score.

example: for agent 1 find and return 2nd 91% and in the next row 9th 91% and in the next row 16th 100%

and just to complicate it some, if there are multiple scores for a date I need al of those scores in consecutive rows (see agent 9)

I would prefer to avoid using macros if possible.

any ideas?
