Formula or VBA code to reformat data

I have some data that was pulled from an SQL server that I am trying to reformat to sort the data into families. I have attached a jpg of the data as it exists and in the form that I am trying to get it into. I tryied using LOOKUP with some nested IF's but because there are multiple lines of the same data (Item Number) I haven't been able to get the formula past the first instance of an ITEM.

    Thanks for the help!!

    Hi,

The attached image is not clear, maybe it will be better if you attach an xl file.
